Accessibility Permits

An application form is available through your physician, from your Town Clerk, the NYS Department of Motor Vehicles, or by selecting this link - Accessibility Permit Application.

  • The top portion of the form must be filled out in its entirety by the applicant and proof of identification will be required.
    • A NYS drivers license or non-driver ID is acceptable.
  • The medical portion must be filled out by the physician and his/her original signature must appear on the application.
  • An applicant can obtain a temporary or permanent parking permit. This is determined by the physician.
  • No fee is involved.
  • • One permit is issued per person. A second permit can be issued to the same person if you do not have handicap license plates. Please request at the time of application
    • If your permit is lost, stolen or damaged, it can be replaced. An affidavit will need to be filled out and signed in order to issue a replacement.
    • If you are unable to come in person, a family member may obtain a permit on your behalf. The form must be properly filled out signed, dated and the applicant’s driver’s license or non-driver ID must be provided.
    • Application is made in the municipality where you reside.
    • Please note – if you reside within the Village of Hilton you can apply at the Parma Town Clerk’s office or the Village of Hilton Clerk’s offices.
